Covid-19 Vaccine Booking Process

1. If you are eligible for a Covid-19 vaccine, please call us to book an appointment. 

The following vaccines are currently offered at the practice (subject to stock availability):

• Pfizer bivalent BA .4/5 booster

• Moderna bivalent BA. 4/5 booster 

• Pfizer paediatric (5-11 years)

If you wish to discuss your eligibility with a doctor, please call to book an appointment.

2. Download and complete the Consent form below. 

3. Return the Consent form to the practice by no later than the day before your appointment.

Please note that patients will be bulk-billed if coming for a covid-19 vaccination only. If you require an additional consultation unrelated to vaccination please inform reception when booking, and please note that fees may apply.
