My Medicare

MyMedicare is a voluntary patient registration model. It aims to formalise the relationship between patients, their general practice, general practitioner and primary care teams, to improve health outcomes and patient continuity of care. 

Register for MyMedicare

Preferably, register via your Medicare App or via your MyGov account.

1.    Log into the Medicare app.

2.    Navigate to 'MyMedicare' under Services and select 'Start'.

3.    Search for Conrad Road Family Practice and then select your preferred GP.

4.    Review and submit your registration.

2.    Navigate to 'Medicare' under Linked Services.

3.    Select 'Register for MyMedicare' and select 'Start'.

4.    Search for Conrad Road Family Practice and then select your preferred GP.

5.    Review and submit your registration.

Alternatively, you can also apply via the downloadable paper form which you can give to our practice team in person.
