My Medicare

MyMedicare is a voluntary patient registration model. It aims to formalise the relationship between patients, their general practice, general practitioner and primary care teams, to improve health outcomes and patient continuity of care. Please click here for instructions on how to register.

Appointments

Standard consultations are booked for 15 minutes. Some consultations require a longer appointment. Please book a long consultation for:

Telehealth 

If your appointment is in person, please call reception when you arrive and remain in your car. The GP will then call you to discuss your symptoms prior to being examined.

Masks
Masks are recommended to be worn by all patients attending the practice as this is a high risk setting.

Appointment Cancellation Policy
Short notice appointment cancellation and failed attendance disrupts service for other patients and adversely impacts the running of our practice. Providing us with a minimum 2 hours notice if you cannot attend your appointment gives us the opportunity to offer your appointment to another patient. 

To cancel an appointment, please call reception on 02 8883 1180. 

Please note that repeated short notice cancellations or missed appointments may result in restriction being placed on your patient record, and you will not be able to make further appointments.

New Patients

Thank you for your interest in attending Conrad Road Family Practice. Please telephone 02 8883 1180 to book an appointment. Initial consultations are booked as a long appointment. 


Please ensure that you have:

a.  Filled out and signed a New Patient form prior to your appointment. Please click here for a New Patient form. If you do not have a printer, please arrive 15 minutes prior to your appointment to complete the form which is available at reception. 

b.  Read the notice for All Patients prior to attending the practice.  

New Patient Appointment Cancellation Policy
To cancel an appointment, please call reception on 02 8883 1180. 

We reserve the right to charge a 'Cancellation/Did Not Attend' fee of $20.00 if you fail to cancel a new patient appointment without giving the practice a minimum of 2 hours notice prior to the scheduled appointment.

Covid-19 Vaccines

Covid-19 vaccinations are now available for eligible patients. For more information, please click here.

Flu Vaccines

Under the National Immunisation Program, government supplied free flu vaccines are provided to certain high risk groups (including but not limited to children aged 6 months to 4 years, and people aged 65 years and over with Medicare). For more information about patient eligibility from the Department of Health, please click here.

Please book an appointment for the flu vaccine if you are eligible for the government-supplied flu vaccine. Please note we have run out of the non-government supplied flu vaccines. 

Patients with Medicare will be bulk-billed for the consult if coming for the flu vaccine only. If you require an additional consultation unrelated to the flu vaccine please inform reception when booking, and please note that routine consult fees may apply.
